Please have an open mind throughout your treatment in our office.
When you first arrive, you will check in at our reception area. If you have not yet completed all of the needed paperwork, we will provide this to you to complete while waiting in our comfortable waiting area. Click here to access our forms page.
Please bring the following items with you to your appointment:
- The patient information form filled out completely.
- The consent form signed in two places.
- HIPAA Signature Page filled out and signed.
- Consent for Pain Medicine Prescriptions. Initial each number 1-19, write down your pharmacy on number 6, and sign at the bottom.
- A referral from your primary care doctor, if your insurance company requires one. Please have your referral before coming in for your appointment; your primary care physician can help you get this. If we do not get this referral we will have to reschedule your appointment or you will be responsible for payment.
-- MEDICARE and Medicare Supplements do not require a referral. --
- Your insurance cards so we may make a copy.
- Any MRI or X-ray films. (If related to your appointment or pain)
- Any medications you are taking in their original bottles.
